ACFE · 2026 REPORT TO THE NATIONS

Fraud Can Stay Hidden — The Losses Do Not Stay Small

The Association of Certified Fraud Examiners (ACFE) examined 2,402 occupational-fraud cases across 143 countries and territories, with more than $3.4 billion in reported losses.
A small recurring loss can become a six-figure problem.
Connect the warning signs sooner—and strengthen the controls that interrupt the pattern.

AP, payroll and financial reporting: different risks

Billing schemes appeared in 21% of cases, with a $90,000 median loss and 14-month median duration. Billing was the second-most-common asset-misappropriation sub-scheme—not the second-most-expensive.

Payroll fraud appeared in 10% of cases globally, with a $63,000 median loss and 13-month median duration. In the US and Canada, payroll appeared in 14% of cases—about one in seven investigated cases, not one in seven companies.

Financial-statement fraud appeared in 6% of cases, with a $1 million median loss and a $4.808 million 75th-percentile loss. It can coexist with other schemes; percentages are not mutually exclusive.
ACFE 2026 · pp. 12, 15, 18, 22, 96 ↗

When people collude, losses rise

One perpetrator: 51% of cases, $55,000 median loss.
Two: 18% of cases, $125,000 median loss.
Three or more: 31% of cases, $324,000 median loss.

Among cases involving co-conspirators, median losses were $200,000 with other employees, $240,000 with vendors and $372,000 with customers. Groups can overlap.

This study concerns occupational fraud—insider abuse, sometimes with outside accomplices. It does not compare occupational fraud with fraud committed entirely by outsiders.
ACFE 2026 · pp. 63, 64 ↗

US & Canada and Western Europe

US and Canada:
  • $110,000 median loss
  • $550,000 at the 75th percentile
  • Billing appeared in 25% of cases
  • Payroll in 14%

Western Europe:
  • $150,000 median loss
  • $1.225 million at the 75th percentile
  • Billing appeared in 21% of cases
  • Payroll in 7%

These are comparisons within ACFE’s investigated cases—not regional fraud rates or the probability that a business will be affected.
ACFE 2026 · pp. 51, 96, 98 ↗

Small and large organizations both need controls

Median losses were:
  • $126,000 for organizations with fewer than 100 employees
  • $123,000 for those with 10,000 or more
compared with:
  • $110,000 for 100–999
  • $100,000 for 1,000–9,999 employees.

These figures show loss severity among the victim organizations studied, not proof that either size group is more likely to experience fraud. Match the controls to your organization’s resources, complexity and risks.

Combine AP Fraud Prevention with Automated Review

A two-pronged approach to AP billing fraud and vendor-data manipulation:
1. understand the risks, red flags and controls
2. put the automated review into action

  • AP Invoice Fraud Prevention teaches the schemes, warning signs and controls.
  • Build Your AP Billing Fraud Dashboard turns defined rules into automated review: scan thousands of invoices and payments in seconds, cross-check approval and bank-change information, and see flagged transactions, rule explanations and supporting data together. Follow connected findings through one investigation and carry the work forward.
  • These two courses complement each other; neither is a prerequisite for the other.

From a Warning Sign to a Practical Response

Learn a way of thinking you can apply when an unusual transaction, a control gap or a pressured reporting request reaches your desk.

1

Recognize

Understand the scheme, reporting pressure or pattern behind the warning sign

2

Check

Look for supporting evidence, legitimate explanations and the control that should have operated

3

Act

Document the issue, involve the right people and follow the appropriate escalation path

4

Improve

Strengthen the workflow, clarify responsibility and make the next review more consistent

Learn Through Cases, Not Just Definitions

Work through invoice and payroll schemes, and reporting failures including Under Armour, Carillion and Wirecard. Connect the cases and relevant US GAAP/IFRS requirements to what you would review, challenge or change.

Add Technology Where It Helps

The prevention courses explain AI and technology-supported controls. The separate Dashboard course is the hands-on build: AI develops and checks the code; you review important results.

Leave with Practical Bonus Materials

Every course includes its own bonus materials or workplace takeaways: red-flag resources, reporting decision tools, or dashboard prompts, fictitious practice data and working versions. Check the individual page for the exact inclusions.

Keep Judgment and Accountability Clear

A red flag is an investigation lead, not proof of fraud. A checklist or dashboard supports your work; it does not replace a professional conclusion.

Become the Person Who Knows What to Do Next

Show measurable improvements: time saved, controls implemented, exceptions found sooner and potential financial exposure identified—not just another completed course.

Add Value in Your Current Role

Implement controls that reduce opportunities for fraud and help catch exceptions sooner. Track what changed: review hours saved, control gaps closed and the value of transactions needing follow-up.

Strengthen Consulting & Fractional Work

Bring a working AP fraud dashboard, a focused control review or a reporting decision framework to your client. Agree the baseline and success measures, then demonstrate the workflow improvement and documented findings.

Build Skills That Carry Forward

Reuse the logic behind schemes, evidence, escalation and control design. Apply reporting judgment under US GAAP and IFRS when pressure arises, and—with the Dashboard—build AI-assisted tools for the next recurring task.

Questions Before You Choose

Which course should I take first?

Start with the risk or responsibility closest to your work.
  • AP Invoice Fraud: vendor, invoice and payment schemes, warning signs, and preventive and detective controls.
  • Payroll Fraud: ghost employees, false hours and diverted payments, connected to HR, payroll and finance controls.
  • Financial Reporting Ethics: for reporting pressure. Review revenue recognition under ASC 606/IFRS 15 and provisions and contingencies under ASC 450/IAS 37. Explore impairment and other estimates, building accounting understanding alongside ethical judgment. Use five practical tools, including Go International for cross-border framework comparisons.
  • AP Fraud Dashboard: build automated checks for 24 red-flag patterns and a recurring investigation workflow.
All four are practical, include course-specific bonus materials or takeaways, and stand alone; there is no compulsory sequence.

What is the difference between AP Invoice Fraud Prevention and the AP Fraud Dashboard?

Think of them as two complementary capabilities:
  • The prevention course explains how AP schemes work, what warning signs to recognize and which controls to strengthen.
  • The Dashboard course is a guided AI build. The tool reviews thousands of connected invoices, payments, bank-account records and approvals in seconds. It applies 24 red-flag patterns, cross-checks supporting information and organizes related findings with rule explanations and transaction-level evidence.
The Dashboard explains the rules it uses; the prevention course supplies the broader control perspective. They are separate courses, not two editions of the same product, and neither is a prerequisite.

Do I need a fraud, audit or ethics qualification?

No specialist fraud, audit or ethics qualification is required. These courses serve a broad range of professionals: internal and external auditors, controllers and CFOs, chief accountants and reporting teams, AP processors and P2P leaders, payroll and HR professionals, consultants and fractional finance leaders.

You may meet the same scheme or ethical dilemma from different angles; practical explanations, cases and takeaways help you connect new learning to your responsibilities. Basic finance, payroll or accounting familiarity is useful. The Dashboard build also requires neither coding skills nor previous AI experience: supplied prompts and detailed instructions guide you.

What practical resources will I take away?

Each course includes its own practical bonus materials and takeaways:
  • AP and Payroll Fraud provide written explanations and red-flag resources for reviewing schemes and controls.
  • Financial Reporting Ethics includes five practical tools for handling reporting pressure, revenue recognition, estimate guidance, ethical sign-off and going international, alongside its 43,000-word text course.
  • The Dashboard includes prompts, fictitious practice files, working tool versions and review guidance.
Use the materials relevant to your role; see each individual page for the exact inventory, which differs by course.

Which course involves building an AI tool?

The AP Dashboard is the hands-on build course. Use a suitable AI service, preferably a paid plan to avoid file and usage limits. AI develops a local HTML tool that opens in your browser without a separate installation or an ongoing AI subscription to run it. You gain both a practical tool and a reusable building skill.

Can I customize the Dashboard without exposing company or client data?

Build and practise with the fictitious course data. The Dashboard course explains safe customization, including the use of sanitized examples, rule settings and mappings while respecting data privacy and company policies. The finished course tool processes imported data locally in your browser. Follow the relevant AI-use, security and storage rules during development and everyday use, and review important outputs when adapting the tool. The course explains all these measures.

Do the courses offer CPE/CPD credits, and will Ethics meet my mandatory requirement?

Each course page explains its estimated credits, instructional basis and completion requirements. Financial Reporting Ethics bases its credit estimate on the text-learning format. The other courses base their estimates on video lessons and assessment activities, as explained on their individual pages

This is general professional ethics training, not a substitute for a board-specific or jurisdiction-specific ethics course. Confirm acceptance with your professional or licensing body before claiming. A Certificate of Completion is available after the relevant requirements are met; these courses do not award a fraud-examiner designation.

Are these courses relevant globally?

Yes. The fraud schemes and control principles apply across borders, while local employment, privacy, reporting and escalation requirements still need to be followed. Financial Reporting Ethics covers both US GAAP and IFRS and compares professional frameworks across several jurisdictions; its Go International tool helps you consider differences in framework emphasis, including situations when local, legal, professional and international rules and "what is accepted in some countries" differ or are in conflict.

Can I use what I learn with my team or consulting clients?

Yes, and I strongly encourage you to put what you learn here into practice — apply the control ideas, bonus tools and reporting frameworks to your team’s or client’s work. Adapt the Dashboard to the agreed fields, rules, responsibilities and review workflow rather than treating every organization as identical.

How does access work, and can my company buy several seats?

You have lifetime access to the course you purchase, with its course updates included. This does not include future separately sold courses or automatic access to the other courses on this page.

Exact materials, current price, tax and refund terms are on the individual sales page and at checkout.
